In participating in the Walk for PNH, I for myself, my executors, administrators and assignees, do hereby release and discharge the PNH Research and Support Foundation and its officers, and all sponsoring businesses and organizations and their agents from all claims of damages, demands, actions and causes of actions whatsoever, in any matter arising or growing out of participation in this event. I hereby consent to and permit emergency treatment, in the event of injury or illness. I also give permission for the use of my photograph in connection with this event for future promotional materials.
